What we are providing
Foundations Early Intervention Services provide in-home and community-based developmental therapy for families of infants and toddlers who exhibit a delay or disability in speech, motor, cognitive, social or other skills.
Refer a Child to TEIS
Complete the online referral form from the state by clicking below. You may also find the form in the MyTN App in the app store on your mobile device.
Please be prepared to provide the child’s name, date of birth, county of residence, parent information (if someone other than the parent calls), and reason for referral.
Developmental Milestones
Skills such as taking a first step, smiling for the first time, and waving “bye-bye” are called developmental milestones. Children reach milestones in how they play, learn, speak, act, and move.
